Plan from the rota, not from an ideal week

- normal working days;
- long days or likely late finishes;

- protected teaching;
- annual leave;
- family and personal commitments;

Only then add study.
Most plans fail because they begin with “two hours every evening” and treat work as an interruption. Reverse it: identify the windows that are reliably available, then assign the right type of study to each.
Use three energy categories:
- High-focus: mechanism learning, difficult calculations, full explanations, substantial timed blocks.
- Medium-focus: mixed MTF questions, error repair, concise concept maps.
- Low-focus: short flashcard review, source organisation, listening to a legitimate educational resource, planning the next block.

Build a weekly floor, target and ceiling
A single ambitious target creates two bad outcomes: exhaustion when you pursue it after a difficult shift, or guilt when the rota makes it impossible.
Instead define:
Floor
The smallest week that keeps continuity during a difficult run. For example:
- two focused 30-minute sessions;
- three ten-question review blocks;
- one 20-minute error-log review;
- one brief weekly plan.

Target
A realistic week under ordinary service pressure. It might include:
- two 60–90-minute learning blocks;
- two mixed question sessions;
- one spaced-review session;

Ceiling
The maximum useful work you will do in a lighter week without borrowing from sleep, exercise, relationships or the following week's capacity. A ceiling prevents “catch-up” becoming unsustainable cramming.
Track completion by week, not by daily perfection. A missed Tuesday session can move to a flex block without the whole plan being declared broken.
The minimum viable study session
On a crowded day, use a session short enough to begin and structured enough to produce evidence.

The 25-minute version
- Select one error-log item.
- Reconstruct the concept with the source closed.

Use anchor sessions and flex blocks
Anchor sessions
Place two or three sessions in the most dependable weekly windows: perhaps a protected morning, a non-call evening and a weekend block. Assign your most important work to these.
An anchor has:
- a start cue;
- one defined outcome;
- materials prepared in advance;
- a stopping point.
“Study pharmacology” is too vague. “Retrieve the core model, verify two weak relationships and answer 20 new statements” is actionable.
Flex blocks
Reserve one or two windows without assigning a topic. They absorb:
- a late finish;
- an unexpectedly difficult question review;
- a missed anchor;
- an urgent Paper A/Paper B imbalance;
- an administrative task before registration.
If the week goes to plan, use the flex block for mixed retrieval or recovery. Do not fill every blank in advance.

Assume one long day and one overnight call. Adapt the sequence; do not copy the times blindly.

The important feature is not the weekday. It is matching task demand to predictable capacity.
Post-call is not a catch-up day
Research on sleep deprivation and residents is heterogeneous, but there is credible evidence of effects in domains including working memory, attention and mood, and studies of resident call rotations have reported impaired working-memory measures. Do not turn this into a rigid medical rule from a blog. Follow your health needs, employer policy and professional judgement. For study planning, use a conservative principle:
- after a demanding call, prioritise safe recovery;
- do not schedule new, high-load material by default;
- if you choose to study later, use a short, low-stakes task;
- never treat lost sleep as a productivity technique;
- move the important block to a recovered window.
An observational study of university students found associations between sleep quality, duration and consistency over the preceding period and academic performance; one night immediately before a test did not explain the pattern alone. The population differs from anaesthesia residents, so the finding should not be overgeneralised. Divide the syllabus by weekly contact
Residents often alternate long blocks of Paper A and Paper B. A busy rota makes this risky because one cancelled week can remove contact with an entire paper.
Use three weekly lanes:
Primary lane
The main weak domain for the week. Allocate the largest anchor.

A smaller domain or linked concept from the other paper. Allocate one medium block.
Maintenance lane

- Resume the next scheduled block.
Do not:
- double every future session;
- clear all overdue flashcards in one night;
- buy a new course to create urgency;
- restart the plan from page one;
- abandon previously secure domains entirely.

If you can take leave, decide its job:
